مقایسه اثربخشی درمانهای شناختیرفتاری و پذیرش و تعهد بر کمالگرایی اختلال شخصیت وسواس جبری

<div style="text-align: justify;"><strong>اهداف: </strong>باوجود شیوع بالای اختلال شخصیت وسواس جبری، در زمینه درمان آن، منابع درمانی کمی وجود دارد. از این رو هدف از مطالعه حاضر، بررسی و مقایسه اثربخشی دو رویکرد درمانی شناختیرفتاری و پذیرش و تعهد، بر بهبود کمالگرایی به عنوان علامت اصلی این اختلال بود. <br>
<strong>مواد و روش ها:</strong> به منظور تأمین هدف فوق، از روش تحقیق شبهآزمایشی به شیوه پیشآزمون پسآزمون با گروه کنترل بهره گرفته شد. جامعه هدف، ۷۳ نفر از مراجعهکنندگان به مرکز مشاوره احیا در سال ۱۳۹۶ بود که دارای اختلال شخصیت وسواس جبری تشخیص بودند و از بین آنها ۴۵ نفر به روش هدفمند انتخاب شدند و به شیوه تصادفی به دو گروه آزمایش و یک گروه کنترل (هر گروه ۱۵نفر) اختصاص یافتند. متغیرهای مورد پژوهش با استفاده از پرسشنامههای کمالگرایی هیل در مراحل پیشآزمون، پسآزمون و پیگیری مورد ارزیابی قرارگرفتند. دادهها به روش تحلیل کوواریانس چندمتغیری مورد تحلیل قرار گرفتند.<br>
<strong>یافته ها:</strong> ننایج حاکی از اثربخشی معنادار هر دو روش درمانی شناختیرفتاری (P=۰/۰۰۱) و پذیرش و تعهد (P=۰/۰۰۰) بر کمالگرایی اختلال شخصیت وسواس جبری بود. مقایسه زوجی گروهها با استفاده از آزمون مقایسههای چندگانه بن فرونی نشان داد درمان پذیرش و تعهد در مقایسه با درمان شناختیرفتاری اثربخشی بالاتری دارد (P=۰/۰۳۵).<br>
<strong>نتیجه گیری:</strong> میتوان نتیجه گرفت که درمان پذیرش و تعهد، به واسطه تمرکزش بر انعطافپذیری روانشناختی مؤثرتر از درمان شناختی رفتاری است. ضمناً پیگیری یکماهه نتایج، نشان از پایداری و تقویت آن داشته است.</div>
<div style="text-align: justify;"><strong>Aims: </strong>Despite the high prevalence of Obsessive-Compulsive Personality Disorder (OCPD), there are few therapeutic resources in its treatment. The purpose of this study is to compare the effects of two therapeutic approaches of Cognitive-Behavioral Therapy (CBT) and Acceptance and Commitment Therapy (ACT) on improving the perfectionism of patients with Obsessive-Compulsive personality disorder.<br>
<strong>Methods & Materials:</strong> This is a quasi-experimental study with pre-test, post-test, and follow-up design using a control group. The study population consisted of 73 patients with OCPD referred to the Ehya counseling center in Rasht, Iran in 2017. Of these, 45 were selected using a purposive sampling method and randomly assigned into two intervention groups and one control group (each with 15 samples). Data were collected using Hill’s perfectionism inventory at three pretest, posttest and follow up phases. Collected data were analyzed using Multivariate Analysis of Covariance (MANCOVA).<br>
<strong>Findings: </strong>The two therapeutic approaches of CBT (P=0.001) and ACT (P=0.000) had a significant effect on the perfectionism of OCPD patients. Pairwise comparison of groups using Bonferroni test indicated that ACT had more significant effect on perfectionism in comparison with CBT (P=0.035). Meanwhile, the one-month follow-up showed the sustainability and improvement of the results.<br>
<strong>Conclusion: </strong>Acceptance and Commitment Therapy (ACT), due to focusing on psychological flexibility, is more effective than CBT in improving the perfectionism of OCPD patients. </div>
