مقایسه عوارض پوستی ناشی از بهداشت دست با محلول مورد توصیه WHO (Hand rub)، صابون آنتی سپتیک و صابون معمولی در پرسنل پرستاری

زمینه و هدف: عفونتهای بیمارستانی از مشکلات و معضلات همه جوامع است و بیشترین راه انتقال آن از طریق دستهای آلوده می باشد. بر اساس توصیه سازمان جهانی بهداشت مبنی بر مصرف محلولهای الکلی جهت بهداشت دست و با توجه به اینکه مدتی است که استفاده از محلول الکلی در برخی بخشهای بیمارستانی مرسوم شده است تحقیق حاضر با هدف مقایسه عوارض پوستی ناشی از بهداشت دست با محلول الکلی مورد توصیهWHO (Hand rub)، صابون آنتی سپتیک وصابون معمولی انجام شده است. روش تحقیق: این مطالعه یک کارآزمائی بالینی سه گروهه تصادفی می باشد. مطالعه بر روی 128 نفر از پرسنل پرستاری شامل پرستاران و بهیاران که با نمونه گیری طبقه ای و بطور تصادفی انتخاب شده اند انجام شده است سه فرآورده محلول مورد توصیه WHO، صابون آنتی سپتیک و صابون معمولی مورد بررسی قرار گرفتند که پس از آموزش بهداشت دست طبق استاندارد سازمان جهانی بهداشت، واحدهای پژوهش بمدت یک هفته از ماده مورد نظر جهت بهداشت دست خود استفاده کرده و در طول مدت مطالعه از نظر بروز عوارض پوستی احتمالی تحت پیگیری بودند پس از پایان هفته، وضعیت پوست واحدها با استفاده از ابزار خود ارزیابی پوست مورد بررسی قرار گرفت. تحلیل آماری با استفاده از SPSS ویرایش 5/14 و با استفاده از آزمونهای کروسکال والیس و من ویتنی انجام شد.
یافته ها: آزمون کروسکال والیس برای بررسی عوارض پوستی در گروهها که شامل چهار جزء ظاهر پوست، تمامیت پوست، وضعیت رطوبت و حس می باشد به تفکیک انجام شد و در بین سه گروه، گروه محلول الکلی در مقایسه با صابون معمولی عوارض پوستی کمتری در مورد تمامیت پوست داشتند(04/0 p=). ولی در مورد سایر اجزاء تفاوت معنی داری مشاهده نشد.
نتیجه گیری: با توجه به اینکه مهم ترین راه انتقال عفونتهای بیمارستانی از طریق دستهای آلوده
می باشد و از آنجائی که با توجه به تحقیق حاضر عوارض پوستی محلول الکلی از نظر تمامیت پوست در مقایسه با صابون کمتر است، با توجه به کاربرد ساده، عدم نیاز به آب و هزینه کمتر، استفاده از آن جهت بهداشت دست توصیه می گردد.
Background and Aim: Nosocomical infections are of the problems of all societies and the most common way for its transference is contaminated hands. According to the recommendations of W.H.O. on using alcohol-base formulation for hand hygiene and it got used to some hospital sections for some time, this study was done with the aim of comparing skin complications of alcohol-based hand rub with antiseptic and plain soap on hands of nursing personnel.
Materials and Methods: This study is a randomized, three-group, clinical trial. 128 nursing personnel including nurses and nurse assistances selected randomly by classified sampling, has been studied. Three hand hygiene products were tested: alcohol-based hand rub, antiseptic soap and plain soap. Each product was used for a week after teaching hand hygiene based on W.H.O. standards. Finally self assessment tool was completed by nurses and during the week skin condition was monitored by researcher assistance. Kruskal Wallis, Man-Whitney and Chi- Square, were used for analyzing data by SPSS software, version 14.5.
Results: Kruskal Wallis test was used for assessment of skin complications in groups with four dimensions: appearance, intactness, moisture content and sensation. Among three groups, the alcohol-based hand rub in comparison with plain soap showed less skin complications in intactness (p=0.04).
Conclusion: Regarding this fact that one of the most important ways of transferring nosocomical infections is personnel's hands, as the alcohol-based hand rub has less skin complications in intactness and less cost in comparison with soap and regarding its easy application even without water, it is recommended to use it instead of soap for hands as an antiseptic.
